if it is a small amount of bleeding and the bleeding volume is <3ml, the patient may not appear comatose. if the brainstem bleeding volume is <5ml, the patient may become comatose within a week of the onset. after active dehydration and intracranial pressure lowering treatment, the patient will regain consciousness. . if the amount of brainstem bleeding is >5 ml, the patient may be comatose for about 2 weeks. if dehydration cannot be actively carried out to reduce intracranial pressure, the patient is very likely to develop foramen magnum herniation, which may compress the heartbeat and respiratory centers, leading to death. brainstem hemorrhage is generally treated conservatively rather than surgically.
the treatment principle for patients with brainstem hemorrhage is to reduce intracranial pressure and control cerebral edema to prevent brain herniation. insist on taking blood pressure lowering drugs to stabilize blood pressure at a safe and ideal level. when there is elevated blood pressure, hypertensive encephalopathy or bleeding tendency, prompt and active treatment should be carried out to avoid cerebral hemorrhage.
