deep coma is the most severe form of consciousness disorder. the symptoms are: bilateral pupils are dilated and fixed, the light reflex disappears, all physiological reflexes, including tendon reflexes and brainstem reflexes, disappear, pathological reflexes cannot be elicited, and the muscle tone of the limbs decreases, showing flaccid paralysis. the vital signs are unstable, manifested by irregular breathing, decreased blood pressure, weakened pulse, high or low body temperature, and a state of imminent death.

a deep coma due to cerebral hemorrhage in an elderly person indicates that the patient's condition is relatively serious. the cause of the patient's deep coma should be found. there are many common diseases such as large-area cerebral infarction or cerebral hemorrhage, brainstem infarction or hemorrhage, hepatic encephalopathy, pulmonary encephalopathy, etc. encephalopathy, carbon monoxide poisoning, hyperglycemia and hyperosmolar state, etc. these diseases may cause patients to suffer from deep coma.
in this case, the patient's bleeding volume can be evaluated through head ct. if the bleeding volume is large, after evaluation, surgery should be considered for treatment if necessary.
however, the clinical prognosis in this case is generally relatively poor. the bleeding site may be lower, such as in the brainstem or cerebellum, or the bleeding site is close to the midline structure, causing the patient's consciousness to be affected. basal surgery is required to remove the hematoma. , save the patient's life and avoid cerebral herniation.
in addition to treating bleeding, the patient's airway protection should also be strengthened, the patient's internal environment should be stabilized, and acid-base imbalance and ion disorder should be corrected. in terms of airway protection, the patient can be given tracheal intubation or tracheotomy, and at the same time it is necessary to prevent patients from developing digestive tract stress ulcers and pulmonary infections.
